Requirements and Training
Personnel File Requirements
- Application Form/Resume
- Copy of Valid ID (Driver’s License, Passport)
- CNA/HHA/LVN Certificate
- First Aid and CPR Certificate
- Physical Exam with TB Test or Chest X-Ray
- Live Scan Result
- Minimum 1 Year Caregiving Experience Required. (Those with no experience are required to attend our in-house induction training.)
- HCA Number. (If you are not registered as an HCA yet, we will help you register with the CA Department of Social Services).

- The above requirements will be needed when you are called for an interview.
- You must complete all requirements to be given a case.
- For First Aid/CPR Certification Training, we offer in-house classes. Please call our office to sign-in and register.
- You may also come and apply directly at our office during normal business hours and bring the above requirements. We are open from 9AM to 5:30 PM, Monday through Friday.
