The 1Heart Circle of Care is how we ensure that our clients and their families get the best home care services possible. The Circle of Care model was developed to align with our company’s goal of clients experiencing zero hospital readmissions. The model comprises seven steps that move in a continuous cycle. The 1Heart Client Care Manager (CCM), assigned to work with your family, will guide you through this process.

Our senior care staff will meet with your family to develop the best possible caregiving plan. Over the years, our Circle of Care model has been awarded honors by professional associations for its high standards of care. In addition, hundreds of families have provided reviews and testimonials of their experiences with our skilled home care services.

How Does the Circle of Care Model Work?

1Heart’s Circle of Care model is the framework that informs all of our services and interactions between clients, families, and our agency. As we progress through the seven steps, the Circle of Care guides all decisions, starting with the initial evaluation and continuing through ongoing evaluation. Our seven steps ensure that all home care programs meet your family’s specialized needs and personal preferences. The qualities clients and families like most about the Circle of Care are its flexibility and adaptability; it is simple to alter services as your family’s needs change. We encourage prospective clients and their family members to read through each step and understand what it entails. This information shows how different 1Heart Caregiver Services is from other senior care agencies!

1. EVALUATION

Our work together begins with an appointment for a 1Heart Caregiver Services Client Care Manager (CCM) to visit you at home. We encourage you to describe your family’s needs and preferences during this meeting. This time together is ideal for asking questions and raising concerns and fears. After the meeting, your CCM will review and use the information to develop your loved one’s home care plan.

2. PLAN OF CARE

After your loved one’s care plan is developed, your CCM will talk with you about our care recommendations. It’s important to note that services included in your care plan can be modified, added, or removed at any time. 1Heart never “locks you in” to a senior care plan that is not beneficial to your loved one.

3. STAFFING

1Heart Caregiver Services’ Staffing Officer will meet with your CCM to suggest the best caregiver(s) for your family member. Recommendations are based on the experience level and ability to meet the personal preferences requested in your family’s care plan.

4. CAREGIVER MATCHING

1Heart Caregiver Services’ Staffing Manager and your CCM will provide professional recommendations for the caregiver(s) that will be assigned to your loved one. You are given the opportunity to review and approve these recommendations and we always welcome your feedback!

5. START OF SERVICE

Once the caregiver(s) have been selected and approved, your CCM will plan a time to finalize the Service Agreement. The payment rates outlined in your Service Agreement depend on your home care care plan. Because this meeting involves financial and legal approvals, families may want to include the care recipient, additional family members, or a conservator.

6. CASE MANAGEMENT

Your CCM is an active manager of your family’s caregiving services and ensures our senior care always aligns with your Service Agreement. Your CCM manages and monitors all assigned caregivers to ensure they follow 1Heart Caregiver Services’ care standards and industry-expected best practices for health and safety.

7. PERIODIC EVALUATION

We fully understand that your loved ones’ physical, emotional, and medical needs will likely change over time. Because of this, your CCM will routinely speak with you to discuss any changes that may be beneficial or required. The 1Heart Caregiver Services staff takes pride in being “part of your extended family.” When your loved ones need additional help, we are available to assist them.
