Our caregivers sit with and monitor a client during their stay at the hospital. When it\u2019s time to go home, the caregiver is there to provide a smooth transition. He or she can transport you from the care facility and provides essential support during an often-stressful time.<\/h6>\n<h6 style=\"text-align: justify;\">The first few days following a hospital stay or a procedure represent a vulnerable and critical time for the patient.
