The Circle of Care features seven distinct and continuous steps, starting with your first evaluation meeting and continuing through periodic evaluation. After creating the care plan, your CCM will review all the details with you and explain our professional recommendations. Don’t forget! The costs and fees detailed in your Service Agreement are for the home care needs agreed upon during the previous four steps. Your family is encouraged to invite the care recipient, additional family members, or a conservator to the Service Agreement meeting as the document has legal and financial implications.

6. CASE MANAGEMENT

Your CCM is an active manager for all caregiver services in your family’s plan. This attention to management ensures all senior care services fulfill the details outlined in your Service Agreement. 7. PERIODIC EVALUATION
