We offer in-home care services for all families with different situations and needs. Our goal is to let your loved one enjoy healthy lives in the comfort of their own home or familiar environment. Through our Circle of Care Program, we tailor our services to your loved one’s unique situation.
Homemaking/Companion Care
We believe that your loved one’s social needs are just as important to address as their health conditions. Our seniors face some emotional challenges when they live alone and these can be difficult to overcome. More often than not, their health conditions limit their ability to drive or travel; thus, making it hard for them to stay connected with friends and friends. Then there is that feeling of loneliness and isolation.
This is where our caregivers can help. For our basic level of care, 1Heart caregivers will provide:
- Companionship and Socialization
- Light Housekeeping
- Meal Planning and Preparation
- Laundry
- Medication Reminders
- Safety Supervision
- Other General Assistance
Hospital Sitters
For those who are staying at a hospital, we also provide sitter services. Our caregivers sit with and monitor a client during their stay at the hospital. When it’s time to go home, the caregiver is there to provide a smooth transition. He or she can transport you from the care facility and provides essential support during an often-stressful time.
The first few days following a hospital stay or a procedure represent a vulnerable and critical time for the patient. This usually involves changes in day to day activities, meal regimens, required medications, rehab exercises, etc. our caregiver will provide monitoring, reminders and emotional support to help the care recipient recover successfully at home.
Once home, the caregiver can provide the following services:
- Errands, grocery shopping, prescription pick-up, housekeeping
- Meal preparation and nutrition
- Rehabilitation reminders, physical and emotional support and companionship
- Transportation to rehabilitation sessions, doctor appointments, and personal event
- Regular status updates to/from the care team and family
Personal Assistance
Basic companion care plus assistance with the Activities of Daily Living (ADLs), such as:
- Bathing and Showering
- Dressing
- Grooming
- Personal Hygiene
- Toileting and Incontinence Care
- Ambulation Assistance
- Transferring
