A CCM (Client Care Manager) will meet with you and your family to determine the best care package that meets your loved one’s needs and budget.

2. PLAN OF CARE

The CCM will discuss the proposed package and ask you and your family about any other care concerns and needs — we want this plan to meet your specific needs and budget.

3. STAFFING

The CCM and the Staffing Officer will review any medical or physical conditions the care recipient has to determine the best caregiver(s) for your case.

4. CAREGIVER MATCHING

5. START OF SERVICE

The CCM and you/the family will review the finalized Service Agreement and the associated rates, which will be based on the selected care package. You are welcome to bring additional family members or a conservator to this meeting.

6. CASE MANAGEMENT

You will have a CCM assigned to your case, so you don’t have to worry about anything going wrong. They will closely monitor every detail associated with your care and keep track of your progress.

7. PERIODIC EVALUATION

As part of the monitoring process, your CCM will visit you regularly to check in and ask about your well-being. If you have any concerns, you can discuss changes you’d like to see moving forward. We always want feedback from the families we serve.
