The 1Heart Circle of Care model is our way of ensuring that families receiving care get the best senior care services possible. This program was developed to support 1Heart Caregiver Services’ goal of zero hospital readmissions. The model contains seven steps that flow in a continuous cycle. A Client Care Manager (CCM) will be assigned to your family to help you through this process.
